Our philosophy of care is centered on ensuring that your needs as the patient come first. For your convenience, you may download & complete our Registration, Health History, and HIPAA forms and bring them with you to your first appointment. Please download the forms first (by clicking right mouse button and choosing "Save Link/Target As" function) before filling out the information.
Since we are a specialty (i.e., referral based) office please do not forget to provide us with any Referral Form from your doctor and/or any relevant x-ray for us to review.
PLEASE NOTE: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
At Pleasant Hill Oral Surgery we make every effort to provide you with premium care and convenient financial options (i.e. cash, debit card, credit cards). Starting January 1, 2023, we will no longer be part of Delta Dental and will be a completely "fee-for-service" practice. This means we are an "out of network" provider for your insurance company. Patients with dental insurance PPO plans are welcome and we will help process your insurance claim(s) as a courtesy to you.
There is a fee for consultation and any x-ray we may need to obtain. Full payment is required at the time of service. To learn more, please call our office for more information.
Our office also offers financing through CareCredit.

BEFORE IV ANESTHESIA
The patient may not have anything to eat for six (6) hours prior to the surgical appointment. Small sips of water is permissible.
If you have an illness such as a cold, sore throat, stomach or bowel upset, please notify the office to reschedule.
If prescribed, please apply numbing cream as instructed and take the anti-anxiety medication one hour prior to surgery.
If you take routine medications, please check with your surgeon prior to your surgical date for instructions.
Do not wear lipstick, excessive makeup, or nail polish on the day of surgery.
Please wear something comfortable such as a T-shirt and tennis shoes.
Contact lenses, jewelry, and dentures must be removed at the time of surgery.
If applicable, all smoking is to be avoided for at least two weeks prior to and after any surgical procedure.
A responsible adult must accompany the patient (before & after surgery), remain in the office during the procedure, and drive the patient home.
The patient should not drive a vehicle or operate any machinery for 24 hours following the anesthesia experience or after any narcotic pain medication use.
BEFORE LOCAL ANESTHESIA or ORAL SEDATION
Have a light meal a few hours prior to surgery.
For oral sedation patients, please come 20 minutes prior to your appointment and have someone accompany and drive you.
Plan to rest for a few hours after surgery with someone available to be with you.
